Sports Medicine - First Year SA Forms Banner

Please read all following instructions and complete each step as laid out. Numbers one through four are required for ALL athletes. Number five is not applicable for all athletes - please read the instructions to see if this step applies to you. 
 
PLEASE NOTE THAT THESE FORMS ARE DIFFERENT FROM, AND SHOULD BE COMPLETED IN ADDITION TO, THOSE REQUIRED BY FAIRFIELD UNIVERSITY STUDENT HEALTH CENTER.  
 
1.     Attached here are the instructions to help you get access our online medical record system - ATS.  Please follow the instructions to create a login, input your medical history information, and fill out necessary sports medicine forms. 

2.     A current physical is required to be eligible for sport participation at Fairfield University.  The physical must be done within 365 days of your move in date to Fairfield.  Attached here is Fairfield University Sports Medicine’s physical form.  Please bring this with you to your doctor’s office to have it filled out and signed by your primary care physician.  After it is signed, this document should be uploaded to the e-files section of your ATS profile.  

3.     Sickle cell trait testing is required by the NCAA for all student athletes.  Results of sickle cell trait testing should be uploaded to your ATS profile as laid out in the instructions packet in step number one above.  Many student athletes were tested at birth for sickle cell trait.  As such, it is recommended that students reach out to their primary care physician to see if a copy of these results can be acquired.  If sickle cell trait testing has not been done previously or results cannot be acquired, a blood test, that can be ordered by your primary care physician, will be necessary to be eligible for sport participation.  See this link if you have questions regarding sickle cell trait testing:  https://www.ncaa.org/sports/2016/7/27/sickle-cell-trait.aspx 

4.      Orthopaedic Specialty Group, P.C. (OSG) employs all of Fairfield Sports Medicine's team orthopedic doctors. As such, ease of communications with the doctors and administrative staff there is of utmost importance. Please download this form, fill out the requested information, and upload the signed copy to your ATS account in the e-files section. If you are not yet 18, please have a parent or guardian sign and date underneath your signature at the bottom of the page.

5.     Attached here is a form required for those student athletes who take medications that contain substances banned by the NCAA.  Common medications include spironolactone, albuterol, and medications used to treat ADD/ ADHD.  If you are taking any substance banned by the NCAA for medical reasons please reach out to your prescribing doctor to help you complete this form.  Please note that it MUST be filled out and signed by the doctor who prescribes these medication(s) for the student athlete to avoid sanctions should he/she get drug tested.  Once this information is obtained the form and all accompanying documents should be uploaded to the e-files section of your ATS profile.  Please see the NCAA for more information: https://www.ncaa.org/sports/2015/1/23/medical-exceptions-procedures.aspx
